Our employees will review your request and forward it to editorial, to the Medical Director, or to customer service. If we decide to integrate your feedback, you will be notified. If you ask a medical question, we will not answer your inquiry and if you provide your name with a medical inquiry, we will delete your name or any identifying information. The information you provide in this form will be kept indefinitely or until you submit a request for the record to be deleted.
